What This Protocol Addresses

Metabolic syndrome — the cluster of insulin resistance, central obesity, dyslipidaemia, and hypertension — requires a multi-pronged approach. GLP-1 receptor agonists (semaglutide, tirzepatide) have transformed the treatment landscape but work best when combined with gut health support, targeted lipolysis, and mitochondrial restoration.

This is an advanced, physician-supervised protocol. GLP-1 receptor agonists require prescription in the UK. Semaglutide is listed for reference; the specific compound should be determined by your prescribing physician based on your individual metabolic profile.

The Four Layers
GLP-1 Agonist
Primary Metabolic Reset
Semaglutide or equivalent. Physician-prescribed, dose-titrated over 3–4 months. The primary driver of weight loss and insulin sensitisation. Must be used with dietary guidance.
AOD-9604
Targeted Lipolysis
Added from month 3 once GLP-1 dose is established. Promotes fat cell lipolysis without affecting insulin or glucose. Complements GLP-1's appetite suppression with direct fat mobilisation.
BPC-157
Gut & Liver Protection
GLP-1 receptor agonists can cause GI side effects including nausea and slow gastric emptying. BPC-157 year-round mitigates gut side effects, supports liver health, and reduces inflammation.
NAD⁺/NMN
Mitochondrial Restoration
Metabolic syndrome is associated with significant mitochondrial dysfunction. NAD+ restoration supports the metabolic shift from fat storage to fat utilisation and improves insulin sensitivity at the cellular level.
Monitoring Requirements

Five metabolic panels across the year: January (baseline), March (GLP-1 titration check), June (mid-cycle), September (progress), December (annual). Core markers: HbA1c, fasting insulin, HOMA-IR, full lipid panel, liver function tests. Weight, waist circumference, and blood pressure at every visit. This is one of the most heavily monitored protocols on Pep IQ — appropriate given the potency of the compounds involved.
